General Information:

1. Always get the name of the department and the name of the person who you spoke to.

2. When mailing information to the trust fund, ALWAYS keep a copy for your records and send a copy to the Local for your personal file.

3. When mailing important documents to the Trust Fund, its recommended that you send it Certified mail with Return Receipt. As the Trust Fund tends to loose documents.

4. It is important that you contact the Trust Fund for any of the following reasons:

-Pension
-Annuity
-Retirement
-Health and Welfare
-Vacation Checks
-Work History Reports

Any questions should be asked to the Trust Fund directly as this is what they are paid to administer.

Divorce

You must send a copy of the divorce decree to the Trust Fund. It does not matter if you were not the one who requested the divorce. You must go to the court house and request a divorce decree and mail a copy to the Trust Fund as soon as possible!

New Children

If you have a child you should send a new enrollment form along with a copy birth certificate and a copy of the social security card to the Trust Fund, remember send this Certified Mail with Return Receipt.

Annual Pension Verification

Each year the Trust Fund sends out an Annual Verification letter to all members that are on the current pension rolls. If you fail to mail this form and documents requested to the Trust Fund, YOUR PENSION CHECKS WILL STOP.

Beneficiary Form:

When you retire from the Trust Fund you should fill out a new beneficiary form with the Trust Fund. Many things change during the years of your membership, such as :

- Grown Children
-Divorce
-Death of Spouse
-New Marriage

And several other changes that may have happened since you last filled out this form.

Death Of A Member:

Contact the Local Office FIRST. The Local will guide and assit you with the procedures.

Change of Address:

When you change your address at the Trust Fund, this information is not shared with the Local.
To change your address at the Trust Fund you MUST submit an "Address Change" form.

Retirement Status:

When you retire at the Trust Fund this does not change your membership status at the Union Hall. You must call the Union Hall to see if you qualify to change your status and lower your union dues.
